The
comprehensive medication review process is designed to improve health outcomes
of Nursing Home residents by promoting the quality use of medicines.
It is a process where the Sydney Medication Monitoring Consultants work
in cooperation with the residents local medical officer(s) to collate and
evaluate medication related information. As is the case with
other professional services, the Sydney Medication Monitoring Consultants
must respect the confidentiality of information acquired about residents
and their families in the course of providing a medication review service.
The
right of residents to make an informed decision as to whether they participate
in this service is acknowledged and therefore reviews are only undertaken
with the consent of the resident (or other legally recognised authority/individual
where the resident is unable to consent). Where residents agree to
participate in a review, it is to be conducted with their full involvement
and in a manner which ensures their information needs regarding the process
and their medications are met.
